You can also manually set the icon, in case you have say two sites in the same domain(i.e. your site, and then the phpMyAdmin directory) by editing opera6.adr.

The images directory and the opera6.adr is in the opera settings folder which is located somewhere in documents and settings on windows and ~your_home_directory/.opera for linux users.

You can use AM Deadlink software to re-download the icon and over-write.

No matte' what I do, I cannot stop my icons disappearing. There only I see stars, sometimes blanks

Opera 9.64 has no such directory named "images" in Opera profile directory (User\Application Data\Opera\Opera\profile).
So, now what? My favicons are fubar.
