The screenshot shows a simple view of a page open in Opera 11 beta with version 1.1 of the extension installed. At the far right of the address bar, after the search box, is the status icon (it's just the letter 'A' in a box). Because there are accesskeys on the particular page, it has a green background.
Although not shown in the image, when hovering the icon there is also a tooltip available that says "accesskeys" or "no" accesskeys depending on whether or not there are accesskeys in the current page. If there were one, the icon would instead have a white background, with red X covering it.
The screenshot also shows the accesskey menu open. It's basically a small floating window that lists the assigned accesskeys (in this case the numbers 1-6) and what they do (in this case, go to a series of pages, which are just listed as raw URLs).
Finally, in my totally unartistic style, there is a red circle around the icon, an a red arrow leading from it to the red circle around the accesskey menu.

The tooltip now says "Accesskeys defined - Shift-esc" or "No accesskeys defined" depending on whether or not there are accesskeys in the current page. If there are none, the current icon has a white background with red X covering it.
The screenshot also shows the accesskey menu open. It's basically a small floating window that lists the assigned accesskeys. Having set in the preferences that I wanted to use the characters 1 2 й ي ц ب z ø they are the ones assigned (the dev.opera page has eight accesskeys defined).
