The documentation mostly needs to be written...

And I already fixed a couple of problems and so version 1.1 is published, and I am about to submit version 1.2

Lots, of course. There are a couple of primary things though...
- Improve documentation
- If you're reading this, it should be obvious why that needs to be done
- RIght now images inside iframes get ignored, due to the way the loading algorithm works. I think the best solution would be for the iframe to send a message to its parent, which can add the available longdescs to its own list. Need to think about the case where the iframe changes though.
- Handle frames
- Because there are still sites that use them. Most particularly, documentation - which is one of the places where people pay more attention to things like accessibility.
- More localisation
- If you want to help, the file to look at is strings.js (it is repeated in multiple locales, so if you want to translate from something other than english go ahead).
