Tamil has a great tutorial on changing the thumbnails... and the background and number of tabs are most easily changed in the latest Opera 10 Beta
Edit: look at the Opera Image Dial Generator as well (tip via kindai)

I think, he used two layers. The top layer is a rectangle, with a solid color(gray) and it has a 1 px stroke with the same color. The layers opacity is 100%, and the fill 10%. The bottom layer is a rectangle with a cutted out circle. The color is white, with 20% oppacity and 100% fill.
