No .idx files generating for certain sites?

I've got a bookmark to Mailinator.com in my bookmarks toolbar. This site doesn't have its own favicon, and thus appears with Opera's generic "star" icon.

I'd like to add my own favicon, just as I've done for many other sites. So... I go to my icons folder to edit the appropriate .idx file -- and none appears to exist there for this particular site. (At least, I'm not finding anything with a "mailinator" search of the folder.)

Do certain sorts of sites not get .idx files generated by Opera? And is there a way I can force this thing to take my custom bookmark?

EDITED TO ADD: I'm now noticing that this is the case for many of the sites for which I have only star favicons: There are no .idx files for them at all.

Oh, and if the answer is, "Well, duh -- of course they don't have .idx files, because they don't have their own favicons" ... then how the heck did I replace these star icons with customized ones in the past?

You'd have to create/borrow the icon and then generate an idx file see this thread.
Update: The more recent Opera versions use png instead of ico files.

Yep, that's what I figured. So I did indeed create some .idx files of my own, with accompany PNG icons. Yet only in some cases is that working. With other sites, it's as if there's a disconnect somewhere -- like Opera isn't "sensing" the .idx file I've made.

For example, I successfully created an .idx file and icon for the site I named in my initial post (Mailinator.com). So now I've got a nice shiny new favicon for it on my toolbar. Success.

However, this is one of the sites where I simply can't get it to work, despite several tries with assorted .idx file names: http://www.thedugout.net

Opera 11.52 1100 on XP SP3, by the way, since I forgot to include that in my original post.

Incidentally, I'm still kind of curious about the question in my initial post: Why is Opera not creating .idx files for certain sites? I can't figure out any common thread among the relevant sites, but maybe the actual smart people around here know better than I...

At first glance wwwdotnet icons seem to have a different naming pattern, the problem is, as with many opera configuration issues, that one has to distill a syntax through a sort of Kremlin-watching.
Some sites simply don't have a favicon, if the idx file is only there to quickly couple an icon to a site it wouldn't be of use.
Opera's way of caching favicons is not perfect, the icons folder can grow like a cancer, idx files seem to self corrupt and cause icons to drop from the bookmarks bar.
