How about a cookie blocking feature that actually works!?

I have seen this behavior with previous versions of Opera as well (with sites such as Google) but haven't really bothered to ask about it. The issue is that the cookie blocking feature seems to fail intermittently.
Or is it my expectations of how this feature works that need to be corrected?
If I have for example Opera set to block all cookies from Yahoo, I would expect my browser to block all cookies. But as shown in below screenshot (if it is not displayed, it can be found at: http://www.imagevimage.com/gallery.php?entry=images/Not_blocking_cookies_(yahoo).png), at least one cookie is accepted anyway.

Another issue that is related to this is (when either one of the "Accept cookies..." options is selected) that the setting "Ask me before accepting cookies" never seems to lead to any prompts asking if user wants a cookie...
How about reporting problems in the right forum — the Opera Browser forum?
I just tested by deleting any yahoo cookies, blocking all cookies, then visiting www.yahoo.co.uk — I still have no cookies from Yahoo.
I then tried the setting "Accept cookies only from the sites I visit" and "Ask me before accepting cookies." The dialogue ppped up numerous times on visiting www.yahoo.co.uk — after declining them all, I still had no cookies for Yahoo.

Actually I took this up years ago as a "problem" but it looks like nothing has changed since then. I do not check the behavior of the sites that I visit every time, and sometimes have just ignored the issue. Partially because bringing it up earlier did not lead to anything and I do not have too much time to put in it. Nevertheless I was surprised to see that the same behavior is still around.
Or perhaps this has something to do with the OS / or the website when accessed here in America...(insert conspiracy theory here)...who knows.
Side note: the image attached to my original post shows Yahoo Norge because I was using Opera Turbo and Yahoo's system assumed I was accessing their site from Norway.

On the general tab for the site preference, what's in the site field? is it exactly "yahoo.com"?
What do you have everything set to at "Ctrl + F12 -> advanced -> cookies"?

FYI my browser is Opera 11.62 running on Windows 7.
My settings in "Ctrl + F12 -> advanced -> cookies" are:
-Accept cookies only from the site I visit
-Delete new cookies when exiting Opera
For google.com, though, I have set the site cookie options (under the Edit Site Preferences) as follows:
-Never accept cookies
(The two checkboxes "Delete new..." and "Ask me..." are greyed out although still looks like being ticked)
Under Site Preferences, the other tabs have the following settings for google.com:
Content: only "Enable content blocking" is ticked ("Enable plug-ins only on demand" is greyed out and ticked)
Display: all checkboxes unticked
Scripting: all checkboxes unticked
Still even right now when I go to google.com, I get cookies from them. So it looks like the cookie-blocking mechanism is not stopping them (or they are doing something like what they did with the Apple Safari browser).
By the way if I press F12 when at Google, the menu that is displayed has "Enable cookies" ticked.
So it looks like if a person wants to disable cookies they will need to do it in two different places..?

In Opera 12.01, I can't confirm that using your settings. All I see in "Ctrl + F12 -> advanced -> cookies -> manage cookies" is "google.com". But, that's just showing because there's a site preference for google.com. There aren't actually any cookies for google.com there. There aren't any listed on the cookies tab in the site preference for google.com either. And, I visited google before testing.
I tried with a fresh profile though. Or,you can delete cookies4.dat in the preferences folder. You might want to delete override.ini in the preferences folder and redo your site preferences too. That all might straighten out your problem. If you turned "delete new cookies on exit" *after* you had some cookies stored though (to save them even after exist), you'll have to do that again.

The option in that menu only shows the global state. It doesn't change depending a site's preferences.

Yes. That's correct. Those settings are inherited from the global cookie setting unless you change them and that's what Opera shows, even if you set "never accept cookies", which then disables those options.
I'd test in a standalone installation of Opera 12.01 to a folder on your desktop so you can see how it works there with a modern version of Opera with a fresh profile. If it works there, then it's just something messed up with your normal profile.
