Extensions are downloaded to the "widgets" folder in your user profile.
On windows XP, this is in: C:\Documents and Settings\Username\Local Settings\Application Data\Opera\Opera\widgets
The "Image Preview Popup" extension by whochan was removed by the author along with all his other extensions after a backlash regarding his in-extension ads. More information here.
