I'm guessing "Windows Firefox" also includes those browsers that use the non-IE flash plugin, including Opera. I wonder if some of the issues attributed to upgrading to Opera 12.00 may actually be related to this defective Flash version.

Flash Player 11.3.300.265 (Mac and Windows, all browsers) was released on July 11th, 2012 to address the following issues:
Windows Firefox Users
- Upload button not working on photoshop.com
- Audio is garbled in Win XP on certain sound cards
- Audio not heard while playing videos in Flash Player on Win 7 and Vista on certain sound cards
- Video not playing for DisneyConnection
- Various general stability issues

Update 7/11/12 - We released Flash Player 11.3.300.265 today to address most of the audio issues that users have been encountering. If you continue to run into sound issues, please try adjusting your sound format using the instructions below.

No audio in some of the speakers in a surround sound system on Vista or Windows 7
Please update to Flash Player 11.3.300.265
No audio on Vista or Windows 7
For users on Windows Vista or Windows 7 that receive no sound at all, please update to Flash Player 11.3.300.265. If this build does not fix your problem, please try the following, temporary, steps.
- Press the Windows key + R.
- Type control mmsys.cpl and press enter.
- Click on the active sound device in Playback tab. (Should be your 5.1 card)
- Click on Properties button.
- In the resulting dialog, go to Advanced tab.
- In the Default Format section, select 16bit 44100 Hz (CD Quality).
- Click on apply and OK.
- Go to IE/Firefox and play any Flash movie.
- Let us know what the result is.
- Also, play around with the Default format combo box in Speaker properties and let us know what works and what doesn’t.
No audio or distorted audio on Windows XP
For users on XP that are encountering no sound, please update to Flash Player 11.3.300.265

I have Flash 11.3.300.270, whereas Adobe says the latest version is 11.3.300.268. However if you go to labs.adobe.com you see they are now working on 11.4, so I guess the build I have will never be officially released.

Today, Flash Player 11.3.300.270 for Windows was released to address a crash that was occurring in the Adobe Flash Player Update Service (FlashPlayerUpdateService.exe). There are no other fixes or changes provided with this build. This release is available for Windows only, and affects the Active X and Plug-in installers, uninstaller, and msi's (available on the distribution page.) No other platforms are affected. Please be aware that this release is not available from the Product Download Center (http://get.adobe.com/flashplayer) which will continue to provide 11.3.300.268. We realize that this might cause confusion for some users. Due to the severity of this issue, we decided to make this build available immediately to help customers affected by this bug. Due to logistical issues and time constraints, we were unable to update the release on the Product Download Center. The next release of Flash Player will correct this disparity. Please note that unless you have been affected by the FlashPlayerUpdateService.exe crash, both 11.3.300.270 and 11.3.300.268 will be functionally identical.

Just noticed Adobe has released 11.3.300.271, which, according to their release note, has some security enhancements as listed in Security Bulletin APSB12-18.
Adobe has released security updates for Adobe Flash Player 11.3.300.270 and earlier versions for Windows, Macintosh and Linux. These updates address a vulnerability (CVE-2012-1535) that could cause the application to crash and potentially allow an attacker to take control of the affected system.
There are reports that the vulnerability is being exploited in the wild in limited targeted attacks, distributed through a malicious Word document. The exploit targets the ActiveX version of Flash Player for Internet Explorer on Windows
