How about mouse type.

I have from time to time had issues if I'm dragging a scroll bar or similar and accidentally tap the RMB while the left is down. No, not really like the "rocker gesture" since you are dragging. Anyway, when I do that then Opera thinks the left button is held down until I let it up and tap it again. This is with a non-Logitech mouse.

It happens on my laptop, with the integrated trackball.

And another thing - it also, but not only, happens when a page has not finished loading -- you want to right click on an image while the page is still trying to load something (sometimes it happens that the page is fully displayed but some obscure junk is still being accessed somehow) and nothing happens. Click the "stop" button and the right-click menu appears (not where you right-clicked, but where the mouse pointer is at the moment, so I surmise the click is not even sent at all in the first place).
But as stated, it also happens "randomly" (no better description, unfortunately), eg, when you want to click a link in an email to open it in a background tab - have to right click 5-6 times before the mnu finally appears.
cheers

i think i got it - after you fullscreen/restore a flashplayer it happens like 90% of time.
to be precise - nearly always with some players, nearly never with others , youtube is ok, some other arent
