'npie_opera.dll' [copy to C:\Program Files\Opera\program\plugins]
'View in IE' and 'View in IE (same tab)' [drag to toolbar]
For anyone interested in this ... keep in mind I have no idea if this actually does anything, or if it might be malware (or merely contain malware), make sure that you check it thoroughly with your antivirus and antispyware. Also keep in mind that if it is what it claims - an Opera equivalent to Firefox's IE Tab extension - that using it does make you susceptible to whatever exploits exist in IE. Since I have no interest in using IE, whether inside Opera or separately, I have no use for this plugin.

Strange, I don't have that option here. Running 10.51, upgraded from 10.10, which has been upgraded many times before.
So maybe this is that "non-standard Opera menu" thing?
Only if I could find a foolproof way to import all my existing customizations (except things like UI/skin things which I can easily re-create), would I consider a "clean install".
Edit: I do have "open with", but only if I click inside an existing tab's content, not if I right-click on a link or the tab itself.

Backup profile
Make a copy of your profile folder and save it to a safe location.
To find the profile folder in your original installation:
Start Opera.
Type "opera:about" in the address field (without quote marks).
In About Opera, under Paths > Opera Directory, the profile folder is listed. A common example is C:\Documents and Settings\Administrator\Application Data\Opera\profile\. The path might also be in two locations (especially for Windows Vista). If so, copy all the contents of the profile folders in to one single folder called "Profile".
After you have backed up your profile:
Locate the VPS folder in your profile folder and rename it to VPS1.
If you use Opera Mail, run Opera with "-nomail" paramaters. Right-click the Opera icon, select Properties from the menu and edit the file name in the Target field to exactly "Opera.exe -nomail" (including a space after exe; without quote marks).
Find the Operaprefs.ini file, rename it to Operaprefs.bak and restart Opera.
Download a clean install
Go to www.opera.com/download.
Click Show other versions.
Click Windows.
Click on the newest version, for example: All languages (Opera 9.51).
Select Classic Installer.
Click Download Opera.
Install Opera in a new folder name, such as C:\Program Files\OperaClean.
In the installer select Use the same profile for all users on this computer. The profile folder is now in C:\Program Files\OperaClean\Profile.
Next steps
Copy over the entire profile folder from your orignal installation to the clean install. See Backup profile above for file locations.
(Edit: I see you just copied this from somewhere else, so here is what I would ask you if you had written this yourself):
I always use the Classic installer, because I don't like per-user profiles.
What is the "VPS" folder?
I don't use email for emailing, but I do use RSS feeds, which seems to use the email database.
You seem to suggest launching the program with email disabled, but for what reason?
And if I am not copying operaprefs.ini to the new install, am I just losing GUI prefs, or am I also losing things like filetype associations, plugin associations, per-site customizations, etc?
