I can confirm this. I've had this too once in a while. I had even prepared some screenshots, but couldn't reproduce it.
BTW, the annoying thing: It was on an embedded ad (!) from an internet bank (!!). Now how stupid can they be?

This is still extremely annoying, similar to the "Open"/"Save" buttons missing on download dialogs. It makes Opera unusable for unexperienced users.
I have noticed that selecting the (still available) "Help" button, and then closing the Opera help window brings the page which had embedded something which required this certificate dialog back to its original (correct) state.
I have reported this as a bug now:
