Cookies issue with Opera 10.53
Issue is as follows:Default settings: Don't receive cookies, don't send Referer
Selected sites: Accept site cookies, send Referer
Opera 10.10 works as expected.
Opera 10.53 don't accept cookies, unless set to accept all cookies.
What the hell? And how to fix it?

I used a similar configuration until 10.51 was released - cookies and javascript turned off globally and enabled per site as required ("accept only from site I visit").
The issue started for me with 10.51 which had the following cookie changes that may be the cause:
Cookies received over NPAPI being incorrectly parsed Reverted the blocking of third party cookies by default Opera sometimes storing a cookie for the domain it came from, rather than the set domain
Since 10.51, I have to select "Accept all cookies" in the individual site settings for cookies to be accepted when global cookies are disabled. Unfortunately, this means that third party cookies are also accepted. To help mitigate this, I use a custom urlfilter.ini that blocks access to most ads and stat trackers. It's not a perfect solution but it works well enough for now.
