Try:
Preferences (Ctrl+F12)
- Advanced
-- Toolbars
And under "Menu Setup" select "Opera Standard"
If that works, then go back to your custom one (assuming you are using one)... exit Opera, go to your ..\Profile\menu\%menusetup%.ini open it, and search for:
Pagebar Popup Menu (the menu when you right-click the tab bar)
Pagebar Item Popup Menu (the menu when you right-click a tab itself)
They should both have sub-items like:
Feature Private, Item, MI_IDM_MENU_NEW_PRIVATE_PAGE="New page, 1,,, "New private page menu""
If those are already there... then fine... if not, then you'll have to add them, which would be best to launch Opera again, go back to Toolbars - Menu Setup... and duplicate the "Opera Standard" setup, then close Opera again, open the file that was just created, and copy/paste the entire sections from "Opera Standard" to your custom config (replacing it's existing sections of the same names)...or copy/paste specific items that you want.
If all of this still fails, then you've somehow disabled Private Browsing... and I'm not sure how...lol

If you are using a custom menu then edit the ini file and remove every section except those which you customized. Opera will load default entires for all those sections that are removed from the ini file. This will help avoiding similar situations in the future. For example i customized image context menu and only sections that i have in my ini file are Image Link Popup Menu and Image Popup Menu(and Version, Info).
