For 10.10 or above I found a tip somewhere on these pages: Close Opera, move the icons folder (In my case c:\Program Files\Opera\profile\icons) to a safe place eg Desktop, just to be sure create an empty icons folder in the profile folder, restart Opera and all should be well.
Drawback is now you have also to recreate your bookmark and bar icons, they can be revived by visiting them (in a few cases I had to remove an icon from the bar, delete the bookmark and drag it up again)
If you have like a few 100 bookmark icons and some 10 searches you could also delete specific search icons AND their .idx counterparts;
for www.google.com delete
http%3A%2F%2Fwww.google.com%2Ffavicon.ico plus
www.google.com.idx
Third possibility if both ico and idx file are present is to repair the idx file: open in notepad,
first line should read "http://www.google.com" minus quotes so delete all past .com
line two should read http%3A%2F%2Fwww.google.com%2Ffavicon.ico (icon filename without path)
Save and you'r done.
