Memang benar Ibu bekerja akan lebih repot mengatur dan merencanakan pemberian ASI, namun apabila kita mengingat manfaatnya, ASI tidak mungkin digantikan oleh susu binatang apa pun termasuk susu formula yang berasal dari susu sapi.

Agar dapat tetap menyusui, ibu bekerja harus merencanakan penyusuan lebih baik. Untuk Ibu yang merencanakannya sejak hamil, pastikan Ibu menyusui dini saat melahirkan dan rawat gabung. Kedua hal ini menjamin pengeluaran ASI yang baik dan penyusuan yang baik. Kemudian berikan penyusuan eksklusif (ASI saja, tanpa cairan lain). Setelah minggu pertama menyusui, mulailah belajar menyimpan ASI dengan cara memerah ASI setelah penyusuan. Segera setelah bayi disusui, ASI diperah (dapat dengan tangan, pompa susu khusus yang mempunyai gelas sendiri untuk menampung ASI, atau pompa listrik yang mulai banyak dijual di maternity shop.

Untuk Ibu yang saat ini masih menyusui eksklusif dan akan bekerja kembali, mulailah dari sekarang memompa ASI setiap selesai menyusui, tampung dalam gelas bermulut lebar, tutupi dengan baik dan simpanlah dalam lemari es. Sebaiknya ASI perahan jangan disatukan, simpan sesuai urutan pemerahan. Cantumkan tanggal sehingga pemberiannya pada bayi dapat diatur (yang lebih lama disimpan lebih dahulu dipakai).

Dari penelitian terakhir di negara maju, ASI dalam lemari es (refrigerator, bukan dalam freezer) dapat disimpan 2-5 X 24 jam, sedangkan dalam freezer dapat disimpan berbulan-bulan ( 3-6 bulan).

Apabila Ibu mulai bekerja, sebelum bekerja susui dulu bayi Ibu. Selama ditinggalkan bekerja bayi akan mendapat susu perah yang sudah Ibu siapkan di lemari es sebelumnya. ASI yang baru dikeluarkan dari lemari es dapat tahan 4-8 jam di udara luar (suhu kamar). Sebelum diberikan, jangan dipanaskan. ASI dalam gelas cukup dihangatkan dengan mencelupkan gelasnya ke dalam wadah berisi air hangat. Kemudian ASI simpanan diberikan dengan sendok, jangan dengan dot, karena akan menyebabkan bayi bingung puting.

Di tempat bekerja, carilah tempat yang cukup nyaman untuk memerah ASI sesering mungkin (idealnya 2-3 jam sekali),. Cukup beberapa menit yang diperlukan untuk memerah ASI apabila ibu telah mahir melakukannya. Jangan sampai payudara mengeras karena ASI terlalu penuh. Apabila payudara mengeras dan ASI keluar menetes, produksi ASI akan segera berkurang karena sel-sel kelenjar yang memproduksi ASI terlalu diregang dan dapat rusak. ASI yang diperah di kantor dapat disimpan dalam lemari es untuk esok hari atau dikirimkan ke rumah.

Saat Ibu pulang, Ibu dapat segera menyusui dan malam hari sambil berbaring (tertidur) Ibu masih tetap dapat menyusui. Setelah bayi minimal 4 bulan (sebetulnya ASI eksklusif dapat dilakukan sampai 6 bulan, tergantung tumbuh kembang bayi Ibu), bayi dapat diperkenalkan pada makanan pendamping. Ini akan mempermudah Ibu menyusui, karena selain ASI sudah ada makanan selingan. Demikian pula bulan-bulan selanjutnya, makanan pendamping ASI akan lebih sering dan lebih banyak jenisnya sehingga Ibu dapat tetap menyusui sampai 2 tahun.

Dengan kemauan dan usaha yang baik serta dukungan seluruh keluarga, termasuk dukungan lingkungan kerja, Ibu bekerja tetap dapat menyusui ekslusif dan penyusuan 2 tahun seperti yang dianjurkan . Hal ini berarti kualitas generasi yang akan datang akan jauh lebih baik daripada generasi susu sapi.

Obesity therapy programme yaitu program holistik, komprehensif terdiri atas behaviour modification, nutritional therapy and dietary therapy, sport therapy dan drug therapy.

Behaviour modification, mengatur jadwal makan secara ketat, tidak mengonsumsi camilan tinggi energi, tinggi lemak, tinggi karbohidrat sederhana dan olahan, seperti camilan gorengan, kue, roti, mi bahan baku tepung.

Nutritional therapy dan dietary therapy diberikan dokter spesialis. Nutritional therapy, diberikan energi 1.200-1.300 kalori per hari. Anda dianjurkan mengonsumsi zat gizi, karbohidrat 60% total energi, lemak 25%, protein 15% atau 50-55 gram, vitamin dan mineral cukup, air 2.500 ml per hari, rendah karbohidrat sederhana dan olahan, rendah lemak dan kolesterol.

Dietary therapy diberikan makanan dengan pola makan 5 kali. Breakfast susu rendah lemak, mid-morning snack buah, lunch makanan utama nasi, mid-afternoon snack buah dan dinner makanan utama nasi.

Sementara itu, banyak penelitian yang membuktikan bahwa risiko kanker dapat diturunkan dengan aktivitas fisik, paling tidak olahraga sudah terbukti menurunkan risiko munculnya kanker payudara, usus besar, uterus dan prostat. Konstipasi atau susah buang air besar (BAB) sebenarnya jarang terjadi pada bayi yang diberi air susu ibu (ASI). Namun setelah masuk makanan tambahan bisa terjadi sang bayi akan mengerang kesakitan ketika BAB. Penyebabnya lebih banyak karena feses (tinja) bayi yang cukup keras dan volume yang banyak.

Untuk bayi di bawah enam bulan, jangan dahulu diberikan obat-obatan pencahar atau feses softener. Alangkah baiknya Pak Ronald meningkatkan serat pada makanan tambahannya (MP-ASI). Dan tidak lupa untuk memberikan banyak jus buah terutama apel.
